PRP services provide individuals with mental health diagnoses and other conditions with a dedicated case manager who helps the individual tackle their life goals which can become overwhelming. This lets the person stay focused on their mental health journey, while also getting support with barriers that might pop up in their lives. PRP complements the work of a primary mental healthcare professional to further their therapeutic plan.swer Number 1

Uptown Lifestyles LLC helps at-risk and underserved individuals achieve their full potential by providing individualized mental health care and life skills support to strengthen clients, families and communities. We do this through a combination of therapy, medication manager and Psychiatric Rehabilitation Program (PRP) services/case management. We presently serve clients throughout Maryland, including via telehealth and in and around offices in Baltimore.

We strive to help clients develop the life skills they need to succeed with their therapeutic plan. The skills we focus on vary with each client, and may include financial management, budgeting, jobs, secure housing, interview skills, access to hygiene and clothing, nutrition and fitness, medication education, computer skills, coping mechanisms, socialization and more. Our goal is always to serve the best interests of the client. Our services are available both on-site and off-site, including in the client’s home.

For many PRP clients, the services we provide are free and covered by Medicaid.

Yes, we are supporting clients via telehealth and video sessions for therapy, psychiatry and PRP/case management services, as needed. Please let us know if you need accessibility support. We are also able to meet clients where they are for PRP/case management.

Each client has an individualized plan to meet their specific needs. Usually, adults will meet with their case manager 6 times per month, and minors will meet with them 3-4 times per month.

What happens during a PRP/case management meeting?
PRP Rehabilitation Counselors/case managers will meet with clients for one to two hours during each visit. We usually like to meet in a location that is comfortable for you – your house or a community setting. During these meetings, the client is an active participant and is responsible for working towards the goals set out in their Individualized Rehabilitation Plan.
